Let’s Talk About Red Light Therapy

I'm working on not having my initial reaction be: "that's probably bullsh!t" when something becomes popular. Red light therapy set off my BS alarm because of the dramatic looking panels on influencers faces and because I had a general idea of how much it cost a clinic to buy a legitimate one and figured there’s no way a home version is good enough.

Then I actually read some research and was honestly kind of surprised. It's not magic, but there's real, repeated, peer-reviewed evidence behind a chunk of what it claims.

What it actually is

Red light therapy (aka photobiomodulation aka PBM) uses specific wavelengths of red and near-infrared light to influence what's happening inside your cells. The leading theory is that these wavelengths get absorbed by an enzyme in your mitochondria (you know; the powerhouse of the cell), which bumps up ATP production — basically, your cells' energy currency. More usable energy at the cellular level is linked to less inflammation, better tissue repair, and in some tissues, more collagen production.

That's the mechanism. Here's what happens when you actually test it on humans.

What the research shows

Pain and inflammation. A 2026 systematic review of randomized controlled trials looked at PBM across chronic pain conditions — fibromyalgia, neck and shoulder pain, diabetic neuropathy, chemo-induced nerve pain — and found consistent, meaningful reductions in pain scores across most studies and very little safety issues. A separate 2025 review specifically on fibromyalgia pooled 17 studies and nearly 900 participants and landed on the same conclusion: it's a legitimate adjunct treatment, not a replacement for everything else.

Skin. This is where the evidence is honestly the strongest for some of the instagram claims, at least so far. A randomized controlled trial found a 30% reduction in periocular wrinkle volume with consistent use, and a 2023 study on skin aging markers found measurable improvements in fine lines, texture, and firmness with red light exposure over several weeks. Heads up: both of these studies are pretty small, so hopefully we’ll see in time with more and larger studies.

Muscle recovery and performance. This is the one I'd tell you to be the most skeptical about. A 2024 meta-analysis of 34 randomized trials found pre-exercise photobiomodulation could modestly improve muscle endurance and reduce some markers of muscle damage — but other trials in the same window found no meaningful benefit for strength or soreness in certain populations. The honest read: possible small benefit, wildly dependent on dose, timing, and who's being studied. Don't buy a panel just for muscle recovery if you’re on the fence about the cost being worth it.

What it's not going to do

It's not going to detox anything (that’s what your liver is for). It's not a substitute for ACTUAL treatment of a real injury, and it's definitely not going to reverse aging or "heal your mitochondria" in the sweeping way some salespeople are saying.

If you want to try it at home

The brand doesn't matter nearly as much as the numbers. Try to ignore marketing and look for three things:

  1. Wavelength. You want red light in the 630–660 nm range (this is the one that does more at the skin level) and near-infrared in the 810–850 nm range (this one gets deeper into muscle and joint tissue). A device that only tells you it's "red light" without a number is a device that doesn't want you comparing it to anything.

  2. Irradiance, not wattage. Total wattage or LED count is a marketing number. What actually matters is irradiance,  measured in mW/cm² at the distance you'll actually be using it. If the company can't tell you that number at 6–12 inches, they either haven't tested it or don't want you to see it.

  3. Dose and time. Research generally lands on roughly 5–20 minutes per area, a few times a week, depending on what you're targeting (skin vs. deeper tissue). More is not automatically better: there's a point of diminishing (and even reversed) returns with light dosing, same as with most things in the body.

A strong consideration: FDA clearance as a Class II device is a low bar, but it's a bar, and it means basic safety testing happened. It's not a guarantee of efficacy, but it filters out the sketchiest options.

Bottom line

I still think a good chunk of the red light therapy market is overpriced hype riding on real science. But "some of this is overhyped" and "none of this works" are different statements. The pain and skin research is solid enough - I'm just telling you to buy based on specs, not vibes :)
